This tut was written for those with a basic knowledge of psp.
I am using the awesome artwork of Ellie_Milk.

Supplies Needed
I used PSPX...but any of them should work.

Xero Filter- Fritillary (Optional)
Monti Blind Mask, get it HERE
Font is Amperzand
Drop Shadow usually V2 H2 50 5
Let's get started
Open the template and hold your shift key down and hit
the letter d to duplicate and close the original.
Remember to add your drop shadow as you go along.
Delete the stars and dotted lines and copyright from the template.
On the red bars, selection, select all, float, defloat and paste paper #6, invert and delete.
You will do these steps on each layer of the template.
Now delete the red bars.
The underlined part remember to do on each section,
I will not be typing that part for each one...
Large black square, paste paper #3 and invert and delete
like before, delete the black square.
Select the red squares now, paste paper #7, invert delete.
Delete the red squares.
Add whatever filter or texture you would like on this layer.
I used xero-fritillary at these settings

Select the black circles, paste paper #6,
you will have to copy this paper twice to fill both circles.
Select the red circles and paste paper #4,
you will have to move it over and copy the paper again to cover the other circles.
The white glitter back, I added noise at 50%.
The black back, paste paper #6, invert, delete.
The gradient back, paste paper #5, invert and delete.
Do not deselect yet, now copy your tube and place either
to the left or right and hit the delete key on your
keyboard. Change the blend mode to Screen.
Duplicate the tube and mirror to the other side.
Now paste your tube and resize if needed.
I had to resize mine some. Place in center and add your drop shadow.
Move the wordart where you would like it to be. I moved
mine just above the edge of the legs on my tube.
Take your magic wand and click inside the white areas of
the wordart and paste paper #10, invert and delete.
Add a inner bevel with these settings
Width 10

50, 32
color white
Then soften the edges with your soft brush tool.
Add flower #5 and place it above the the black circle
layer on each side.
Take ribbon #1 and rotate 90% to the left and place
at the bottom, just above the tube down by the legs.
Duplicate and flip and position above the red circle layer, duplicate and mirror.
Elements, lipstick2, lips3, ipod and shoe 2,
resize to 50%. Place the lipstick and shoe on the left side
of your tag.
Heart 2 resize to 30% and place on the left toward the top of tag, refer to my tag for placement.
Rotate the ipod free rotate, 20% to the right,
place on right side of tag. Take your free hand selection,
draw around the window part of the ipod, add a new raster layer, fill with color of choice, move below the ipod, do not deselect, add your tube, with the face part showing, then invert and delete, deselect now. Change the blend mode to soft light.
Add the lips on the right side also.
Take paint #1 and resize to 125% and place behind all layers at the bottom part of the tag, then
duplicate and flip, mirror and position at the top part of the tag.
Once happy with everything, then merge visible and resize your tag.
Now the mask, I saved it for last, because we needed to
resize the tag first and then use your pick tool to pull
and stretch to where you want the mask to be.
Now add all your copyright and name and you are done.
